Sending an annual Post-Placement Report is the responsibility of every adoptive parent of a child from Cambodia. Two of the documents required in the dossier (for anyone who adopted after March 2001) were Post Placement Agreements - one for the Dept. of Foreign Affairs and one for MoSalvy.
The agreement stated:
.... these documents were both signed and notarized. The importance of sending these reports can not be over-emphasized.
At this time there is no standard Post-Placement Report form. If you would like help putting your report together, click here for a suggested format. Don't forget to include 6-8 pictures of your child on 1-2 color copied pages (note: Cambodians believe it is unlucky to be photographed in groups of 3... and that the smallest of the three in the photograph will have bad luck. This is good to keep in mind when choosing pictures to send to Cambodia) If you adopted through an agency, your report can be sent to Cambodia through them. If you adopted independently (or have lost touch with your agency), you can send the report directly to: (Note: These names/addresses are the most up-to-date, verified in September 2004, after the seating of the new Cambodian government) His Excellency Hor Namhong An additional copy of the report should be sent to: The Royal Cambodian Embassy in Washington, D.C.
